ABSTRACT
Objetivo: Analisar a percepção da deformidade estética resultante da tenotomia da cabeça longa do biceps (CLB) de acordo com o grau de especialização do profissional assistente. Métodos: Foram analisados120 pacientes submetidos à cirurgia artroscópica do ombro.Os pacientes foram fotografados e colocados em uma apresentação PowerPoint, sendo esta submetida à apreciação por três especialista sem cirurgia do ombro, três ortopedistas generalistas e três residente sem ortopedia do terceiro ano de graduação. Resultados: A amostra geral dos pacientes observou-se uma maior concordância entre os especialistas em cirurgia do ombro. Na amostra dos pacientes com tenotomia da CLB, os especialistas concordaram entre si de forma moderada. Na amostra dos pacientes com IMC ≥ 30 Os ortopedistas tiveram uma concordância pequena. Entre os cirurgiões especialistas e médicos residentes não foi verificada concordância. Conclusões: A percepção da deformidade estética resultante da tenotomia da CLB não apresentou concordância significativa entre profissionais especialista sem cirurgia do ombro, ortopedistas generalistas e médicos residentes,muito embora os especialistas demonstraram uma percepção semelhante da deformidade em pacientes tenotomizados. Os profissionais especialistas também demostraram maior concordância quando da avaliação de pacientes com IMC<30. A avaliação dos pacientes obesos baixou o nível de concordância entre os três diferentes grupos de profissionais. Nível de Evidência III. Estudo Caso-controle.
Objective: To evaluate the perception of an esthetical deformity resultant from arthroscopic long head biceps (LHB) tenotomy according to the degree of experience of the assisting professional. Methods: 120 patients submitted to shoulder surgery were photographed and photos were mounted in a PowerPoint presentation. Three shoulder specialist surgeons,three generalist orthopedic surgeons and three graduate dresidents analyzed the presentation. Results: On all patients we observed most agreement among the shoulder specialists. When just the patients with LHB tenotomy were analyzed, the specialists agreed moderately, the generalists had small agreementand the residents, a poor one. Analyzing patients with BMI < 30, there was major agreement between the specialists, while the generalists and residents had poor agreement. Analyzing patients with BMI ≥ 30, the generalists had small kappa agreement, while the specialists and residents had no agreement. Conclusions: The perception of an esthetical deformity regarding a LHB tenotomy did not have significant agreement between different level of professionals, eventhough the specialists showed similar perception on tenotomy patients. The evaluation of obese patients lowered the agreementon the three groups of professionals. Level of Evidence III.Case Control Study.
